RVA offers the Social Prescribing service to patients who have been referred by GPs so they can access community support for their non-medical needs. Many patients referred by GPs at Tilehurst Potteries surgery share common issues including social isolation and loneliness, anxiety and loss. Tilehurst Together will offer fortnightly group support for patients to develop more connections and learn how they can improve their general wellbeing. Topics for the group could include relaxation and mindfulness, coping with bereavement and making healthy lifestyles choices.
This is part-time post is for 10 hours per week, over a 12 month period.
The project co-ordinator will:
-Work with a steering group to develop a programme of talks, activities and discussions which help to address the needs of local people
-Develop a simple measure to evaluate the impact of the project
-Take a community development approach to enable local people to build their confidence and skills and gradually lead/organise the ongoing programme of activities
The ultimate aim of the project is a sustainable range of local activities which help to improve people’s wellbeing and connections.
Salary: £25,000 pro rata
